A 15-year old boy presented with subcutaneous emphysema after an asthmatic attack. Chest X-ray showed no evidence of pneumothorax. Computed tomography (CT) scan revealed the presence of air into the spinal canal associated with pneumomediastinum (Fig. 1) . The pathway of air from the posterior mediastinum to the epidural space through the right intervertebral foramen was demonstrated. The patient had an uneventful recovery. Repeat CT scanning showed spontaneous resolution of the epidural emphysema.
