A 52-year-old-male was scheduled for video-assisted mitral valve reconstruction. Preoperative TEE revealed a polypoid tumor on the ventricular side of the fully competent aortic valve.

To gain access to the left ventricular outflow tract, the base of the anterior leaflet was incised to obtain view of the aortic valve (Figs. 1 and 2).
