Traumatic disruption of the aortic arch

A 54-year-old man presented with a decreased mental status and hypotension after a rollover motor vehicle crash. He suffered thoracic, cerebral, and orthopedic injuries in addition to a traumatic disruption of the aortic arch (Fig. 1A) . Surgical repair of the aorta was delayed four weeks, to allow for stabilization and/or improvement of his intracranial and pulmonary injuries. A 3D reconstructed chest computed tomography (CT) scan (Fig. 1B) revealed a transverse tear on the anterior wall of the aortic arch associated with a large pseudoaneurysm near the takeoff of the innominate (brachiocephalic) artery. The injury was repaired under deep hypothermic circulatory arrest without complications. A 3 month follow-up CT scan demonstrated an intact aortic repair.
