Left atrial thrombus on a Teflon patch for ASD closure

A 68-year-old man who underwent atrial septal defect closure with a Teflon patch and mitral annuloplasty in 1977 was referred to our department for dyspnea. Echocardiographic findings revealed a large thrombus of 12 cm2 (Figs. 1 and 2) extending into the pulmonary veins originating from the atrial septum. The thrombus was approached by a modified Guiraudon incision and excised. The defect was closed by a Gore-Tex patch.
