Remodeling after right pneumonectomy

A 56-year-old male underwent right pneumonectomy for hilar lung cancer. After 12 months he was doing well but he had a chest CT-scan for other reasons. Images showed an extreme shift of the mediastinum with herniation of contralateral lung (Figs. 1 and 2) . The patient does not complain of any symptom of postpneumonectomy syndrome.
