This is an interesting, carefully controlled animal study comparing two crystalloid solutions for lung preservation. Storage times have been taken to an extreme (24 h) and perfusion following storage to 7 h. Interestingly, one animal died in each group each exhibiting re-perfusion injury. This aside we are given a body of evidence suggesting that Celsior (originally constituted for heart preservation) is superior to LPD. I would suggest that further work is required on two accounts, (1) to support this observation and (2) to tease out a mechanism. Phillipe Menasche planned his Celsior solution to promote anaerobic metabolism during the anoxic period. It may well be that preservation of energy stores is as important in lung transplantation as it is in preservation of the myocardium. I would be very pleased to see comparisons of Celsior lung preservation against our own preferred colloid and prostaglandin based method. This work from Hanover is a constructive step towards obtaining a reliable and effective lung preservation strategy.
