A 55-year-old male suffered acute thoracic pain, vertigo and hypertension. Due to emergent symptoms, emergency surgical repair was performed as distal aortic arch replacement under hypothermic circulatory arrest and antegrade cerebral perfusion, proximal descendens aortic replacement, and subsequent bilateral subclavian artery bypass in a single procedure (Figs. 1 and 2).
