A 69-year-old woman was admitted to our hospital because of chest pain. Her physical examination was normal apart from tachycardia. First electrocardiogram revealed ST segment depression. Coronary angiography showed many fistulae between the three main coronary arteries and the left ventricle (Figs. 1 and 2 and Videos 1 and 2).
