Key Words: Aneurysm • Right atrium • Congenital • Cardiac

A 13-year-old asymptomatic male presented with the incidental finding of cardiomegaly (Fig. 1 ). He was operated upon on prognostic grounds (risk of thromboembolism and arrhythmias). The aneurysm was excised and the right atrium repaired resulting in a normal size chamber (Fig. 2 ). The patient remains asymptomatic a year later.

Fig. 1. Right atrial (RA) aneurysm presenting as cardiomegaly (A), found to be 8 cm in diameter by transthoracic echocardiography (ECHO) (B) and 400 ml in volume on MRI (C and F). RA reduced to 3.76 cm after surgery as seen in transoesophageal ECHO (E) resulting in a normal chest X-ray (D).

LA = left atrum, LV = left ventricle, RV = right ventricle, AO = Aorta, PA = pulmonary artery, CPB = cardiopulmonary bypass.
