Coronary artery spasm after mitral and tricuspid annuloplasty

A 75-year-old woman, with normal coronary arteries (Fig. 1 ), underwent uneventful mitrotricuspid annuloplasty. At ICU, sudden electrical and hemodynamic instability occurred. Echocardiography showed ventricular dysfunction without valve dysfunction. Coronariography demonstrated diffuse vasospasm (Fig. 2 ). Hemodynamic stabilization was obtained after intracoronary nitroglycerine and intra-aortic balloon pump. She made a full recovery.
